NICE's fertility guideline was fully updated on 31 March 2026 (NG257, replacing the older CG156): instead of the previous rigid split — 3 cycles under 40, 1 cycle for ages 40–42 with extra conditions — NICE now recommends access up to age 42 based on ovarian reserve testing and clinical judgment, and for the first time explicitly extends eligibility to same-sex couples. This is clinical guidance, not a funding mandate — individual ICBs still set actual funding levels and can offer less than NICE recommends.
In many England ICBs, same-sex female couples and single women are still required to self-fund 6–12 rounds of IUI before qualifying for NHS-funded IVF — sometimes called the "gay tax." NG257's eligibility update doesn't compel any ICB to remove this requirement, since ICBs set local funding policy independently of NICE guidance.
Private IVF: base packages commonly quote £4,000–£7,000, but this usually excludes medication (£1,000–£2,500) and add-ons like ICSI (~£1,375) or embryo freezing (~£550 plus storage) — a realistic all-in total is often £6,000–£10,000+ per cycle. Private IUI runs roughly £800–£1,800 per cycle, with medication billed separately.

Legal considerations

Only HFEA-licensed clinics can perform IVF/IUI with donor sperm or eggs, and anonymous donation has been banned since April 2005 — donor-conceived people can request identifying information about their donor from age 18.
Undergoing IVF/IUI at a licensed clinic doesn't by itself determine legal parenthood for a non-birth partner — that depends on consent forms signed before treatment (see the Known Donor guide).
NHS funding rules and cycle limits change periodically and differ by nation and, within England, by Integrated Care Board — always confirm current criteria directly with your GP or local board before assuming what applies to you.

NHS-funded cycles cost little to nothing directly to the patient where you qualify, but eligibility varies enormously: Scotland (up to 3 cycles), Wales (2 cycles), Northern Ireland (1 cycle), and England (typically 1 cycle, varying by Integrated Care Board). Outside NHS funding, private IVF realistically costs £6,000–£10,000+ per cycle once medication and common add-ons are included, and private IUI £800–£1,800 per cycle with medication separate.

Timeline

Weeks 1–4: referral (via GP) and fertility workup at an NHS or private clinic; for donor-sperm treatment, choosing a donor through the clinic or an external, HFEA-approved sperm bank.
Per cycle: IUI is typically a single-day procedure timed around ovulation; a full IVF/ICSI cycle runs roughly 2–4 weeks from stimulation to egg retrieval and transfer.
Multiple cycles are common — the fact that Scotland, Wales, and NICE's NG257 guidance all recommend more than one cycle's worth of access reflects that success often takes more than one attempt.
